WHERE ARE THE INCISIONS FOR UPPER- AND LOWER-LID BLEPHAROPLASTIES?

The upper-lid blepharoplasty incision is located approximately 6 to 8 mm above the upper-eyelid eyelash crease. It is used to remove skin, a strip of muscle tissue from the orbicularis oculi muscle and in many cases can be used to access bulging upper eyelid fat. Shown here in pink, it can also be used for surgery to correct a droopy upper eyelid in some cases.

The lower-eyelid blepharoplasty can be conducted through an incision immediately below the lower-eyelash line. This incision, shown here in green, can be used to remove redundant skin and bulging eyelid fat, as well as to smoothen the junction between the lower eyelid and cheek.

Alternatively, an incision can be made on the inner surface of the eyelid, shown here in purple. This incision is invisible on the outside (note the dotted line in the figure). It can be used to remove bulging fat while smoothening the junction between the lower eyelid and cheek. If extra skin is present, it can be tightened with a CO2 laser or removed with a second incision immediately below the lower eyelashes.
